Nethack 3.0 is a big update from 2.2+, changing a lot of things about the game.
Doors can now be closed, or even locked as opposed to the always open state in previous versions.
You may end up with a kitten companion instead of a dog companion- you'll see an 'f' instead of a 'd'.
Color is a much bigger part of the game now, with colors representing the thing more often now, like gold, and now there can be different colors for the same symbol.
There's now an oracle, who will provide you with hints and tips for the greatly changed lower levels (which now can be a static level focused on solving a puzzle, instead of randomly-generated).
The configuration on this version is also much simpler than previous versions.
